2. Performance rating in the latest three rating periods (if applicable) ;
3. Certified photocopy of Certificate of Eligibility/Report of Rating;
4. Certified photocopy of valid and renewed PRC License/ID;
5. Certified photocopy of certificate of Employment, Contract of Service, or Duly signed Service Record;
6. Certified photocopy of certificate of Trainings/Seminars Attended;
7. Certified photocopy of scholastic/academic record, e.g. Transcript of Records (TOR), including
graduate and post-graduate with certification;
8. Checklist of Requirements and Omnibus Sworn Statement, and
9. Other pertinent documents.
Submit application letter and documents in five (5) sets (with original copy of each document) at the
Schools Division Office through the Records Section.
